What are the contents required in a petition for arbitration?

A petition for arbitration must include the following:

    • The date of the petition;
    • The name and address of the the parties; name(s) and address(es) of the witness(es) (if any);
    • A summary of the dispute; • Grounds and evidence (if any);
    • Specific request of the claimant and the value of dispute;
    • Name and address of the person whom the claimant selects as an arbitrator or requests for designation as arbitrator.
