What are the required contents for a petition?

The petition must include the following:

    • Day, month, and year that the petition is made.
    • Name of the Court receiving the petition.
    • Name, address, and workplace of an individual plaintiff or head office of a plaintiff that is an agency or organization; telephone number, fax number and email address (if any).

If the parties have agreed on an address for contact by the Court, that address shall be clearly indicated in the petition.

    • Name, address, and workplace of the individual whose rights and interests are to be protected, or the head office of the agency or organization whose rights and interests are to be protected; telephone number, fax number and email address (if any).
    • Name, address, and workplace of an individual defendant or head office of a defendant that is an agency or organization; telephone number, fax number and email address (if any). If the address, workplace or head office of the defendant is unknown, the last address, workplace or head office of the defendant shall be indicated in the petition.
    • Name, address, and workplace of the person with related rights or obligations, or the head office of the agency or organization with related rights or obligations; telephone number, fax number and email address (if any). If the address, workplace, or head office of the person with related rights or obligations is unknown, the last address, workplace or head office of such person or organization shall be indicated in the petition.
    • The alleged infringed legitimate rights and interests of the plaintiff must be contained in the petition; specific matters requested to be settled by the Court in the dispute with the defendant or the person with related rights or obligations.
    • Full name(s) and address(es) of witnesses (if any).
    • A list of documents and evidence enclosed with the petition.
